2015 Recreational Program Late Registration Process
We have currently closed our open registration period however are accepting late registrations for a limited number of openings per age group.
Registrations will be confirmed based upon space availability at the time we receive the completed registration form and payment. If we are unable to confirm an immediate space your registration will be placed on a waitlist and assigned to a team in the order the registration was received.
During this period registration is by mail only.
Check payments will not be deposited until a team assignment is confirmed.
Please submit the following:
- Signed Registration Form, two pages must be completed in full.

- Payment of $245.00 per player (Check made to: NYSA)

At the point your registration is confirmed you can expect to hear from your Coach regarding a team assignment.

Recreation Fees Refund Policy
- Refund Request Deadline is August 15, 2015. Requests received after the deadline date will not be accepted.
- Refund Requests received March 1, 2015 – May 31, 2015. A refund of the registration fees, less a $25.00 processing fee will be issued. This is a refund of $170.00 per player.
- Refund requests received June 1, 2015 – August 15, 2015. A refund of the registration fee, less a $75.00 processing fee will be issued. This is a refund of $120.00 per player.
- Any late fees assessed are not refundable.
- Approved refunds will be processed within 30 days of receipt.
